Welcome To The Practice



The NHS in Heywood, Middleton and Rochdale, in collaboration with MSD, a pharmaceutical and healthcare company and Verily, a health technology company, is developing a new project that aims to improve care for people who have, or may develop, long term health conditions. Wellfield Health Centre is participating in this project.

Using “non-identifiable” patient data, the project aims to test new ways to support those people most at risk from chronic ill health. To start, the project will focus on diabetes, heart failure and chronic obstructive pulmonary disease (COPD) which includes conditions such as chronic bronchitis and emphysema. Your data will only be used in the project if you have one of these conditions.

There is no obligation to participate in this project. If you would rather not take part, please fill in and return the form which you will find in the patient leaflet available at reception or click this link to download it from here. This will be used to ensure that your data will be excluded from all future analysis. You can choose to opt out at any time.


Patient Access to Online Services

You can now book appointments, order repeat prescriptions and view aspects of your medical records online. To get your log in details you will have to provide photo I.D. and proof of address and fill in a consent form. This needs to be returned to the practice along with your I.D.

To save time you can download the consent form by following the Patient Access Link on the left or collect one from the reception desk. Once we have received your information we will be able to give you your log in details.

SMS Text Reminder Service

We are introducing a new SMS Text Reminder Service. This service will send appointment confirmations, remind patients about booked appointments and allow patients to cancel appointments, all by text message.

In the future we also plan to use the SMS service to send invitations for annual reviews (if you have a chronic condition) or Flu vaccination invitations.

In order for us to utilise this system effectively, please make sure we have an up to date mobile phone number for you. This service is not compulsory and patients can choose to ‘opt out’ simply by letting us know your preferences.


Health Checks

We are now running Health Checks for eligible patients. If you fit the relevant criteria and age range you may receive a letter or a phone call inviting you for an appointment. These appointments are just to assess your general health and wellbeing such as height, weight and blood pressure, and to ask a few lifestyle questions. Due to an NHS decision, from the 29th February 2016 we can no longer accept Hospital Only Prescriptions. We apologise for any inconvenience.
